New Patient Information
Are you interested in becoming a new patient of the Center for Pain Management? Please read through the below information and then contact us at 317.706.7246 and ask to speak to a New Patient Coordinator.
Some key questions to which we will need answers:
- Were you referred by a physician? If so, by whom?
- Have you been seen by a pain clinic/center/physician before? If so, where, by whom and when?
- Have you been discharged or released by another pain center? If so, when and why?
- What kind of pain are you experiencing?
- When do you experience pain and for how long have you been in pain?
- What type of insurance do you have?
After you talk to our New Patient Coordinator, you may be asked to download and read through our New Patient Welcome Packet. This packet includes a list of information we need from you and your referring physician(s) to be able to successfully admit you to our medical practice. The forms in this packet are required to be filled out before your scheduled appointment time. Once you fill them out, you can mail, fax, or bring them along with you to our office for your appointment
Some other information that may be of interest to you:
- Insurance Carriers: We provide coverage for a variety of commercial insurance plans and HMOs. However, you are ultimately responsible for understanding and following your insurance program and its requirements.
- Medical Records: We must receive certain medical records before we can see you as a patient.
